Health Assessments

Our program increases the completion of health assessments and wellness exams. Our health care providers act as an extension of care in the home or office where we can help identify gaps in care.

Our health visits gain deeper insight into the environmental and social issues affecting your employees' health. Our team of clinicians work collaboratively with you to capture appropriate documentation and our in-house, certified coding team performs chart reviews for all visits.

Transition of Care Program

Improve care coordination and reduce readmissions
Our Transition of Care Program provides the opportunity to extend care into the home by helping your employee after a recent hospital visit. We help those who have been admitted to the hospital and have a strong likelihood of a hospital readmission based on diagnosis, co-morbidities and functional health status.

How Our Transition of Care Program Works
Our clinicians work directly with an employee's PCP to ensure care coordination needs are addressed to keep employees safe in their homes and out of the hospital.

Once an employee has returned from the hospital, a claim is submitted. Alegis Care works on behalf of your business to identify and assist at-risk members.

Chronic Condition Programs

We offer two types of chronic condition programs.

  1. Chronic Care Management (CCM) - We act as the employee's PCP
  2. Complex Care Program (CCP) - Employee has a PCP and we augment their chronic care plan in the home

How Our Chronic Condition Programs Work
We work with you to identify employees that require additional in-home care management for their chronic conditions. Our team identifies any possible health and social issues to develop an appropriate plan.

Our health care providers and nurses live in the communities they serve and have developed relationships with additional key health resources, such as:

  • Specialists
  • Diagnostic labs
  • Home health agencies
  • Hospice
  • Social agencies
