ENSURING MEMBERS WITH ADVANCED ILLNESSES RECEIVE APPROPRIATE CARE

Our Advanced Care Program provides compassionate care and support through prevention and relief of physical, emotional and spiritual pain for members facing advanced illnesses. We work with the member’s primary care physician (PCP) and specialists to provide an extra layer of support tailored to meet individual needs and improve the quality of life.

Advanced Care Program Collaboration

We take special care to understand and respect the different physical, psychological, social, spiritual and cultural needs of your members and use a team approach to help address their needs. Our team includes physicians, nurses, social workers and a chaplain who work closely with the member’s PCP. The team meets regularly to make sure members and their families have a complete plan in place to help meet their goals. All treatment will be in accordance with the member and family’s wishes.
