Prior to an in-home visit, Alegis Care will call to:
  • Let you know what to expect during the in-home visit
  • Tell you about how long the in-home visit will take
  • Verify your address
  • Verify the best contact number
  • Ask your date of birth for verification
  • Provide the name of the medical professional visiting you in your home
  • Schedule a date and time that works for you
  • Confirm your in-home visit the day before
Here's what you can expect during an in-home visit:
  • We will do an assessment to understand your medical history, current medical conditions and medications
  • We will develop a plan to help you stay out of the hospital
  • If you are hospitalized, we will check-up on you when you return home
  • If necessary, we will assist you with scheduling your next visit, answer any questions you may have and update your doctor on how you are doing
