General Summary of Position

The Supervising Physician will provide clinical oversight for non physician practitioners.  This will be a hands-on management position that will involve understanding and meeting the needs of patients and payer referral sources, as well as HP office staff and clinicians.  The position will be directly responsible for general oversight to the day to day clinical issues that arise at Home Physicians including coverage for providers who are out, communicating and help resolving issues with vendor partners.  The position will also help on board new providers and work on improving current and future Home Physicians standard operating procedures and policies.

 Essential Job Functions:

  • Chart review of non-physician providers.
  • Cover for absent providers. Specifically order review and sign off, renewing medication, reviewing labs, responding to phone calls.
  • Provide in office expertise for medical questions/situations that arise where a field provider is unable to address quickly.
  • Develop Vendor relationships and process improvement.
  • Interface with patients/referral sources, Home Physicians clinicians and departments, and Chicago/Baltimore management.
  • Perform “ride along’ with existing and new Home Physicians employees.
  • Ongoing evaluation and development of policies and training procedures.

Mandatory Performance Requirements

  • This position is primarily an administrative/in-house supervisory role; however, travel for business or patients may be required less than 20% of the time.   
  • Must guarantee and follow a commitment as to availability.
  • Notify the office about cancellation and missed visits that day.
  • Participate in Mandatory Provider Meetings.
  • Respond quickly to all phone calls within 24 hours and emergency calls within 30 minutes
  • Turn in paperwork within 48 hours.
  • Be willing to be part of telephonic “call" pool.

Experience/Education: 
    
5 years work experience required.  10 years total clinical work experience, preferred.
Board Certification or Board Eligible
Valid driver’s license and/or dependable transportation necessary.
Active licensure in the State of Maryland (or in the process of obtaining licensure.)
Additional licensure in Delaware, District of Columbia, or Virginia highly desirable.

OBJECTIVE

RN Case Manager will join a team of associates who will provide guidance to our members regarding their health care needs as part of our Chronic Care Management and Skilled Nursing Facility Management Programs

OVERVIEW

As a clinical front line leader for our ALEGIS CARE Chronic Care Management (CCM) and Skilled Nursing Facility(SNF) Programs,you will be responsible for coordinating clinical and non clinical care for our patients.  This will be performed in conjunction with other health care providers inside and outside of the organization.  You will be responsible for the review of both actual and proposed medical care and services against established clinical protocols and program guidelines.
 

SPECIFIC DUTIES TO INCLUDE

  • Monitor calls.
  • Track performance.
  • Report trends.
  • Conduct CCM and SNF program analysis and make recommendations for improvement.
  • Implement appropriate actions to control trend.
  • Create and conducts provider outreach initiatives.
  • Ensure compliance with regulatory requirements.
  • Recommend services for Health Plan members utilizing care alternatives available within the community and nationally.
  • Recommend alternatives if appropriate using your knowledge of clinical protocols.
  • Identify members for specific case management and / or disease management activities.
  • Taking after hours call to be available for patients and assist providers.
  • Monitor case management activities, post-discharge calls, discharge planning and pre-assessment of elective admissions.
  • Ability to interact with a member in their living environment.
  • Proactively screen patients for changes in their medical status.
  • Develop non hospital alternatives of assistance to patients
  • Proactively address end of life wishes and expectations with patients and family members.
  • Other duties as assigned

Medical Director - PODIATRY

Home Physicians/Alegis Care is one of the largest physician services companies, 100% focused on providing medical care and coordination in the home.  We have over 12,000 active patients in our two current primary care markets (Chicago metropolitan area and Baltimore/DC).

The medical director is a podiatric physician supervisor who manages other podiatrists.  This physician leader performs clinical oversight activities, including case review, documentation audits and in person clinical supervision.  The medical director is a clinical educator and consultant, staying up to date on medical and surgical management.  The medical director participates in customer meetings, quality assurance and improvement activities, and other committees or tasks as assigned.  The medical director is responsible for delivering the highest possible quality medical care to the patients managed by the medical director's staff.   This is a full time administrative position.

Major Responsibilities:

  • Direct supervisor for assigned staff, including performance management activities, staff   development, orientation and training activities.
  • Works to achieve quality medical care and outcomes for patients, in a cost-effective manner.
  • Creates and maintains "on call" coverage schedule.   
  • Improves clinical outcomes through daily interactions  with podiatrists using  education and excellent  written and verbal communication skills.
  • Provides direct daily clinical oversight for new providers until such a time as oversight can be decreased.
  • Participates in quality processes such as medical documentation audits, inter rater reliability actions and quality projects.
  • Serves as a mentor or coach to other podiatrists  with ongoing quality and performance improvement processes. 
  • Improves podiatry  relations   through direct communication, knowledge of appropriate evidence-based clinical information, and the fostering of positive collegial relationships with medical colleagues.
  • Addresses customer service issues   with mentoring and support from leadership staff.
  • Investigates and responds to client or stakeholder questions to assist in resolving issues with mentoring and support from leadership staff.
  • Achieves customer satisfaction and regulatory/accreditation agency compliance goals by assuring timely and complete documentation, including documentation  of appropriate clinical measures and metrics.
  • Provides clinical insight and management support to other functional areas and matrix partners as needed or directed.

Position Requirements and Qualifications:

  • Experienced  podiatrist  with a current unrestricted medical license.
  • Residency training and board-certification preferred.
  • Exhibits ethical and professional behavior.  .
  • Preferred:  Prior physician leadership positions.
  • Preferred:  Experience in medical and surgical  management in the home environment plus knowledge of HEDIS and Stars measures and managed care experience.
  • Computer Competency:  Microsoft Office - Word, Powerpoint, Excel competency, EMR utilization, on line medical research.
  • Ability to work within a changing business environment and balance patient advocacy with business needs.
  • Experience with managing multiple projects in a fast paced matrix environment.
  • Demonstrated ability to educate colleagues and staff members.
  • Successful experience with change management.
  • Demonstration of strong and effective teamwork, negotiation, conflict management, decision-making and problem-solving skills.
  • Successful ability to assess complex issues, to determine and implement their solutions, and to resolve problems.
